The Halliwell Surgery
The Halliwell Surgery houses 3 General Practices operating from purpose built premises, opened in 1993.
We are situated in the Halliwell area of Bolton, in close proximity with the town centre.
Our Practice has 1 full time doctor (male), and two part time doctors (1 male and 1 female), two part time Practice Nurses, one Practice Manager, seven reception staff and one Trainee Assitant Practitioner.
Dr Uddin, Dr Anwar and Dr Munawar are the GP Partners of the Practice. Dr Uddin has been with the practice since February 2012, Dr Anwar since March 2013 and Dr Munawar since April 2014. Dr Uddin and Dr Munawar are both male GPs and Dr Anwar is a female GP.

DR ANWAR'S MATERNITY LEAVE
Our GP Partner, Dr Anwar, will be going on maternity leave from 01 December 2014 for 6 months.
Dr V Bramwell, another lady GP, will be covering her maternity leave during this time.
DR HUNT'S RETIREMENT
Dr Hunt retired from the practice on 30 March 2014. He will continue to see patients as usual until that date confident that Dr Uddin and Dr Anwar will ensure continued good medical care and practice.

We are looking to start an online Patient Participation Group to look at ways in which you can influence the quality of service you are provided with. We will initially be asking patients to participtate in a survey to identify our strengths and weaknesses and then, in turn, work together to discuss ideas and any planned changes.

WE WILL SOON BE REQUIRED TO SUPPLY YOUR PERSONAL AND CONFIDENTIAL MEDICAL INFORMATION TO THE HEALTH AND SOCIAL CARE INFORMATION CENTRE (HSCIC). THIS INFORMATION WILL BE TAKEN FROM THE PRACTICE IN A FORM THAT CAN IDENTIFY YOU. THE PRACTICE HAS NO CHOICE BUT TO ALLOW THE HSCIC TO EXTRACT THIS INFORMATION. HOWEVER, INDIVIDUAL PATIENTS CAN INSTRUCT THEIR PRACTICE TO STOP THE TRANSFER OF DATA.
