Appointments are for one person and separate appointments need be for each person wishing to see the doctor. If you have a number of things to discuss please ask for a double slot. Request a Home VisitPlease phone 01204 463111 before 10-30 am. A home visit is for patients who cannot get out to the surgery or elsewhere. We need patients co-operation in coming in whenever possible. Obtain Test ResultsThe doctor or nurse may give you individual instructions about a specific test result. Back to top Order an ambulanceIf you need an ambulance to take you to an out-patient hospital appointment, this is now arranged by the Referral and Booking Management Service(RBMS). As soon as you receive your hospital appointment, please ring RBMS on 01204 462888 and they will arrange the ambulance for you. Ambulances for any follow up appointments at the hospital will be arranged by the hospital. Back to top Chase up a hospital appointmentWhen the doctor refers you for a consultant opinion, he will give you a Choose and Book letter with a unique booking number and password. All referral appointments are arranged via Choose and Book and you will be asked to ring and confirm your hospital appointment. The receptionist will explain the procedure to you before you leave the surgery. Should you have any questions regarding your appointment or waiting times, Choose and Book are available for you to ring on 01204 462888 Back to top Out of Hours Emergencies  The NHS has set up a new telephone 24 hour confidential helpline called NHS DIRECT. It is staffed by trained Nurses who can give immediate information and advice on medical problems. NHS Direct can also "direct" you to the Royal Bolton Hospital or Bolton Medical Services. The Number to ring is 0845 4647.
Our existing out of hours service will remain unchanged. If you require medical advice when the surgery is closed, phone the surgery. An answer phone will instruct you to ring Bolton Medical Services (386655), who will then contact the Doctor on call. Please do not abuse this emergency service with minor complaints which could wait to the next available surgery.
Alternatively, a walk-in centre is now available for the treatment of minor injuries and simple conditions. This is situated in Lever Chambers Centre for health on Ashburner Street in the town centre and is open between 8am and 9pm Monday to Friday and 8.30am and 4pm at weekends. Back to top Register with this PracticeIf you wish to register with this practice you will need to bring your medical card into the surgery. This will need to be signed inside. If you are unable to find your medical card, we will supply you with a registration form to complete. From then on, you will be registered with this practice and your medical records can than be requested from you previous doctor. Because this can take some time, you will be offered a new patient medical with one of our Practice Nurses. She will take details of relevant medical history that your new doctor will need to be aware of any repeat medication you are taking. Please note that you will not be able to request repeat medication until you have seen either the nurse or the doctor.
You will also be given a practice leaflet, which has our telephone numbers and details of the surgery times and clinics at the surgery. Back to top Speak to the doctor/nurseIf you wish to speak to the doctor or nurse you will need to ring the surgery and speak to one of the reception staff. The receptionist will ask you: - Who you are
- Why you need to speak to the doctor/nurse. If the reason for speaking with the doctor/nurse is confidential, you must say so to the receptionist, who will respect your reason for confidentiality. If you feel comfortable to tell the receptionist the reason for your call, she can inform the doctor.
You will normally be asked to ring at a certain time to speak to the doctor and this is usually at the end of a surgery. If you feel that it is urgent for you to speak to the doctor/nurse, the receptionist will speak to the doctor/nurse in between patients and a message will be given to the doctor/nurse requesting that they call you back. Back to top Request a Travel VaccinationIf you are going abroad for your holidays you will need to find out from your travel agent if it is advisable for you to have any vaccinations. This varies depending on your destination. The Practice Nurse is also able to advise you on which vaccinations you may need.
Prior to making an appointment with the Practice Nurse for travel vaccines you will need to ask the receptionist for a form that you need to complete to clarify what vaccinations you may have already had. Once you hand this form back to the receptionist, she will send this information down to the doctor who will then decide whether or not you need any vaccinations. If you do need some vaccinations please make an appointment with the practice nurse.
There is not usually any charge for these travel vaccinations, although, if you need a certificate verifying that you have had certain vaccinations (usually meningitis) there is a £7.50 charge.

Back to top Yellow Fever Vaccination CentreWe are an approved Yellow Fever Vaccination Centre, please ring reception (463111) to make an appointment. This is not covered on the NHS so there is a charge. The charge is currently £50.00 in line with current recommended charges. You will also be provided with an International Certificate.
