When using our online services, for accurate identification of your records, we require you to enter details of your name, date of birth, address and telephone number.
 
We also offer you an option to enter your e-mail address.
 
This page explains how we handle that information and the implications of either entering or not entering your email address.
 
We respect and protect your confidentiality and privacy. This system does not store any information about you. It merely forwards by email to the surgery the appointment or prescription or other details that you have selected and the identifying details about yourself that you enter.
 
If you enter your email address the system will also send you an email confirming the details of your prescription request or any other details.
 
If you use a shared computer or email address and you do not want others to know that you have made a request etc. then do not enter your email address.
 
Email to the surgery confirming your information/prescription request carries your email address as the sender. If this fails to be delivered to the surgery for any reason then you may receive the "bounce" notice from the mailserver and you will be alerted to contact the surgery to confirm your online request. Unfortunately bounce messages from the NHS messaging service are rather brief and cryptic, but at least you will be made aware that something has gone wrong.
 
In the case of prescription orders which "bounce" you may either re-order online or use the normal procedure for requesting a prescription (i.e. by post or by bringing your request into the surgery).
 
Note that this arrangement is simply a failsafe in case of lost emails - in normal operation you shall receive an email confirming your information change or prescription request.
 
You should also be made aware that, if you use the web browser that has an "autocomplete" facility then this may reveal to others that you have used the system since, if they enter the first letters of your name, then the browser may prompt them with your full name. If this is of concern to you then please do not use the system but use the normal procedures for ordering prescriptions or making amendments.
 
When using the online services you must be aware of the above points.
 
If you do not accept them at all then do not use the system.
 
If you are happy with them apart from your e-mail confirmation then do not enter your email address.
 
If you prefer to receive an e-mail confirmation and/or to have the failsafe of having the surgery e-mail returned to you in case of delivery failure, and you are not concerned by the points above then please enter your e-mail address when requested.
