Make an Appointment

  • All our surgeries are run by appointment only. 
  • Appointments can be made for the Doctor or Nurse by telephoning the surgery between 8.30 and 6pm on Tel number 08444 771 751 
  • You may also call into the surgery to make an appointment. 
  • Please note that one appointment is for one person only, separate appointments must be made for other family members. 
  • If a patient needs to be seen urgently and no routine appointments are available you will be offered the next available emergency appointment. 
  • If your appointment is no longer required please telephone the surgery to cancel so it can be offered to someone else. 

Order a Repeat Prescription

Repeat prescriptions are issued for medication which your Doctor has agreed you need to take on a regular basis. These are usually issued on a two monthly basis, but you will need to see your Doctor periodically so that your treatment can be monitored.
 
New patients will need to be seen by the Doctor before any current medication can be issued.
 
You may request medication using any of the following methods:

  • Use the order form attached to your prescription, tick the items you require and hand it in at reception. 
  • Use the order form attached to your prescription, tick the items you require and send it through the post enclosing a SAE if you require your prescription posting back to you, or you may collect it yourself at the surgery, or arrange for someone else to collect it for you. 
  • If you do not have your order form you may write down the items you need including the name of the drug, the strength how often you take them and how many you have. 
  • Alternivately you can order your prescription using our online services on the website. Click onto online services and follow the instructions 
  • Using the online service from this web site. 
  • Please give 48hours notice (this is from the day that we receive your request and does not include weekends. 
  • Prescriptions may be picked up between the hours of 8.30am and 6pm Monday to Friday.

Request a Home Visit

Please try to attend the surgery if at all possible.
However if you are too ill to attend please telephone the surgery on Tel Number 08444 771 751 before 10.30am whenever possible. Please give brief details of your problem to the receptionist, so that the Doctor can assess the urgency of the visit.
 
You may be visited by any of the Doctor's usually after the morning surgery unless your problem is more urgent.
Please remember, three or four people may be seen in the surgery in the time it takes to visit one patient, also the Doctor is better equipped in the surgery than he is from his bag.

Obtain Test Results

If you have had tests such as blood tests, urine tests, cervical smear tests X-rays etc, you would be contacted by the surgery if the result was abnormal., however if you wish you may contact the surgery to obtain your results.
The Doctor or Nurse will be able to tell you how long your results will be as this differs between different tests. It is practice policy that the results should be given to the patient with the exception of minors. If there is any reason that the patient is unable to request the results his/herself, written consent must be given by the patient to a named person.
 
You may contact the surgery after 11am Monday to Friday to obtain your result.

Order an ambulance

If you need transport to take to the hospital for an appointment please arrange by telephoning the surgery on Tel number 08444 771 751 after 11am Monday to Friday.
 
You will need to tell the receptionist which hospital you are attending, the department and time of your appointment and the date.
 
You will also need to tell them whether you will need a two-manned ambulance,(this will be necessary if you need assistance to get into the ambulance, you will also need to let them know if you need a wheelchair. In certain circumstances, if medically necessary, a patient can take an escort with them.
 
Please ensure that this service is for people who genuinely need hospital transportation and must not be abused.
 
Please ensure you give the surgery at least three working days notice, otherwise we will not be able to arrange transport for you.

Register with this Practice

If you wish to register with this practice you will need to call into the surgery and put your request in with the receptionist.
 
She will be able to provide you with a registration form to fill in some details, your name, address, date of birth and your former address if different from your present address, and the name and address of your previous Doctor.
 
Once you have completed this form you should hand it in to the receptionist, she will give you a practice leaflet and an appointment with the Doctor or nurse for a brief medical check, details of your current state of health will be discussed, also details taken of any medication you are currently taking.
 
This is because it can take a while before we receive your medical records from your previous Doctor.

Speak to the Doctor/Nurse

If you wish to speak to a Doctor or Nurse you will need to telephone the surgery and speak to one of the receptionists. She will ask you:

  • Who you are 
  • The reason you need to speak to the doctor or nurse. 
  • If the reason for you wanting to speak to the doctor or nurse is confidential you will need to tell the receptionist who will respect your reason for confidentiality. If you feel comfortable to tell the receptionist the reason for your call, she can inform the doctor.  

You will normally be asked to ring at a certain time, this is usually either before or at the end of a surgery. If you feel that it is urgent to speak to the doctor or nurse the receptionist will speak to the doctor between patients and you will be asked to ring back in a certain amount of time.
 
All the receptionists are trained how to deal with telephone calls of life threatening or of a very urgent nature.

Request a Travel Vaccination

If you are travelling abroad for your holidays you may need some vaccinations. This varies depending on your destination. Any of the Practice Nurses will be able to advise you which vaccinations you will need and will also be able to discuss any other concerns, or health issues you may have regarding travelling abroad.
 
When you make an appointment you will need to tell the receptionist that you require travel vaccinations
 
There is not usually any charge for travel vaccines, though if you need Yellow Fever. there is a £45 charge, this is for the vaccination and the certificate.

Out of Hours

You could first contact NHS Direct on 0845 46 47 where you can speak to a highly qualified Nurse. Alternatively, you should ring 386655 - your call will be answered by a receptionist at Landmark House, the out of hours service in Bolton.
